Person-centred disability support

Good Support Starts With Understanding the Person

Needs matter. So do preferences, routines, goals, interests and the person's own choices.

One & Only takes time to understand the individual and shape support around real life rather than treating every participant the same.

Support shaped around individual needs

Working with families and allied health professionals

Five principles

How We Think About Support

1. Listen Before Acting

Understand what the participant wants and needs rather than assuming.

2. Support Participation

Where appropriate, help the person take part in everyday activities rather than automatically taking over.

3. Respect Choice

Treat participant preferences and goals as meaningful parts of support planning and delivery.

4. Work With The Wider Support Network

Where relevant, communicate with families, Support Coordinators and allied health professionals so day-to-day support reflects agreed recommendations.

Principle five

Stay Honest About Fit

We only accept support that falls within our qualifications, experience, registration and ability to safely and appropriately meet the person's needs. Saying no to work that is not a good fit protects the person receiving support, and it is a decision we are willing to make.

Active support

Helping People Take Part, Not Automatically Taking Over

If someone is learning or maintaining an everyday skill, doing the entire activity for them may be faster. Faster is not always the point.

Where appropriate, we support participation through prompting, practical guidance and repeated opportunities to take part. The level of assistance reflects the person's abilities, needs and goals.

This approach is not suitable in every situation, and we do not apply it as a blanket rule.

Collaboration

Day-to-Day Support Should Connect With the Bigger Picture

One & Only has experience working alongside families, Support Coordinators and allied health professionals.

If recommendations or strategies have been provided, support workers need to understand how those recommendations relate to daily activities. That is the difference between a strategy existing on paper and a strategy actually being used.

  • Understand the recommendation and what it means in practice
  • Apply it consistently across day-to-day support
  • Raise changes in the person's needs with the relevant people

We do not provide allied health treatment. We apply relevant recommendations within our own scope of support.

Funding

Support Should Have a Clear Purpose

One & Only aims to use participant funding appropriately and accurately, so people receive support that relates to their needs and agreed plan.

Pricing is as applicable under the current NDIS pricing arrangements for the supports provided.

We do not provide financial advice, manage NDIS plans, provide Support Coordination or control participant budgets.

How it works

Start With a Conversation

Tell Us What Support You Are Looking For

Complete the short consultation request, or call if that is easier.

  2. STEP 2

    We Talk Through What Matters

We learn about the person's needs, goals, preferences and current situation.

  3. STEP 3

    Discuss the Right Next Step

If the services and situation are a suitable fit, we discuss the appropriate support and what happens from there.
