What this website collects
The consultation form on this website collects the information needed to begin a conversation about support: who the enquiry is for, the kind of support being looked for, what would make the biggest difference, a name, contact details, a suburb and postcode, a preferred contact method and an optional short message.
What we do not ask for
We do not ask for a diagnosis, NDIS number, medical history, detailed health information, funding amounts or sensitive personal records through this website. Please do not upload or include those details in the form.
How enquiry information is used
Enquiry information is used to contact you and to discuss support needs. Participant records, progress notes and support documentation are not stored on this website.

Retention, access and complaints
Enquiry information is retained only as long as needed to respond to you or to meet NDIS and record keeping obligations, then securely destroyed. Participant support records are retained for the period required under NDIS Practice Standards.
You can ask us at any time for a copy of the personal information we hold about you, or ask us to correct it. We respond within a reasonable time and free of charge.
If you are unhappy with how your information has been handled, contact us directly and we will acknowledge your complaint and respond in writing. If you are not satisfied with our response you can contact the Office of the Australian Information Commissioner, or the NDIS Quality and Safeguards Commission on 1800 035 544.
This website does not use advertising or tracking cookies. We use basic, privacy respecting analytics to understand how pages are used, and we do not sell or share your information with third parties for marketing.
